2/5/2023 EnchantaVaughn Approved 1. Behavior: Client exhibits a poor attitude when transitioning from social setting to therapeutic setting. Client displayed hostile and disruptive manners toward members of her family. Client did not start her therapy right away, but instead gave everyone a hard time and continued to pout and complain about simple tasks. Mom suggested client talk to her counselor and stop letting small stuff get to her so that she can have a better day. Client was observed avoiding processing her feelings and remained in a negative mood. 2. Behavior: Client was encouraged to lose the attitude and participate before she ruins her chance to get free help. Client have struggled to take her therapy serious and takes her counseling and treatment for granted by wasting time and being disrespectful toward mental health professional. Mother reports these are the same behaviors she is getting in the home and school setting. 1. Intervention: QMHP separated client from inappropriate family interactions, in efforts to reduce tension and stress-provoking situations in the home environment that seemed to start problems between her, her mom, and younger brother. QMHP encouraged client to resist negative behaviors in efforts to refrain from requiring to be redirected by her mom so many times. QMHP provided understanding of mom’s perspective of her disciplinary actions and why she is there to help resolve any issues related to her mental health. 2. Intervention: QMHP supported client with working on herself and trying her best to understand the purpose of each session. QMHP reminded client of ISP objectives. QMHP reviewed examples of alternative appropriate behaviors she could engage in without being so disruptive. QMHP encouraged client to refrain from displaying negative behaviors towards her mom, to talk calmly, and be mindful of how she responds so that it is clear what her intentions are and where her heart is. 1. Response: Client modeled how to separate herself from negative situations so that she doesn’t continue to be the problem but a part of the solution. Client understood the importance of listening and following directions and have reduced time spent outside to learn more about ways to handle her home obligations and support her family. Client utilized relaxation strategies. Client reported a decrease in symptoms following the relaxation techniques. 2. Response: Client was acceptive of redirection given by her mom and counselor. Client recognized her triggers and was able to identify common things that she used bother her. Client was observed strategizing a plan to better herself by writing down her thoughts and feelings in a journal and exploring her creative side with arts and crafts. Client was taught to take deep breaths until she no longer feels upset or the need to take her anger out on anyone else.
